ABSTRACT

Objective:To investigate the effect of personalized biofeedback therapy for anorectal dynamics and defecation dysfunction in patients with rectal cancer after restorative resection, and to explore the useful method to prevent low anterior resection syndrome.Methods:Rectal cancer patients in hospital from January 2017 to June 2018 were enrolled in the present study, who were going to undergo sphincter-preserving surgery. They were randomly divided into control group and intervention group. Participants in the control group received routine nursing, while the intervention group carried out personalized biofeedback therapy for three months. After 1th month, 3th month, 6th month of followed-up, the anorectal dynamics index such as anal resting pressure, rectal resting pressure, maximum systolic pressure, initial rectal volume, rectal fecal sensory capacity, rectal maximum tolerance capacity were compared between two groups, in addition, the defecation function were also evaluated.Results:Preoperative, after 1th month of followed-up, the anorectal dynamics index was no significant difference between intervention group and control group ( P>0.05). After 3th month of followed-up, the rectal resting pressure was significantly decreased [(6.65±1.70)mmHg vs (7.65±1.72)mmHg] and initial rectal volume, rectal maximum tolerance capacity was significantly increased in the intervention group compared to the control group [(27.18±4.21)ml, (114.59±23.72)ml vs (20.78±5.76)ml, 100.65±17.73)ml] (all P<0.05). After 6th month of followed-up, the rectal resting pressure was significantly decreased [(4.54±1.24)mmHg vs (5.80±1.30)mmHg] and anal resting pressure, maximum systolic pressure, initial rectal volume, rectal fecal sensory capacity, rectal maximum tolerance capacity was significantly increased [(34.06±8.95)mmHg, (147.07±32.08)mmHg, (31.10±5.80)ml, (45.77±8.74)ml, (130.99±25.67)ml vs(29.12±7.02)mmHg, (128.60±27.26)mmHg, (26.23±5.39)ml, (40.92±7.21)ml, (109.17±25.02)ml] in the intervention group compared to the control gorup (all P<0.05). There was no significant difference of defecation function between intervention group and control after 1th, 3th month of followed-up ( P>0.05), however, the defecation function was better in the intervention group compared to the control group ( P<0.05). Conclusion:Personalized biofeedback can significantly improve the anorectal dynamics of patients with rectal cancer after restorative resection, promote the defecation dysfunction syndrome, which is worthy of popularization and application.

ABSTRACT

OBJECTIVE: To investigate the effects of Kangfuxin liquid on repairing cartilage defect model of knee osteoarthritis (KOA) in rabbits and its mechanism. METHODS: Totally 72 male New Zealand rabbits were selected and randomly divided into model control group and Kangfuxin low-dose, medium-dose, high-dose groups, with 18 rabbits in each group. A cartilage defect model of the medial femoral condyle of the right knee joint in rabbits was established by drilling after anesthesia surgery. Then the rabbits in each group were given medicine via articular cavity immediately. Kangfuxin low-dose, middle-dose and high-dose groups were given 20%, 40%, 80% Kangfuxin liquid; model control group was given constant volume of normal saline consecutively, 0.2 mL/kg, once every 3 days. At 4th, 8th, 12th week after medication, the wound repair of cartilage defect in rabbits was observed. Immediately after medication and at 4th, 8th, 12th week after medication, repaired tissue of cartilage defect in rabbits was scored histologically with Wakitani scoring standard under light microscope. At 12th week after medication, pathological changes of repaired tissue of cartilage defect in rabbits were observed by Masson staining. The levels of NO, SOD and LPO in joint fluid and PYD in urine of rabbits were detected by ELISA. RESULTS: At 4th, 8th, 12th week after medication, compared with model control group, cartilage defects in rabbits were repaired well in Kangfuxin low-dose, medium-dose and high-dose groups. At 4th, 8th, 12th week after medication, compared with immediately after medication and model control group at same time point, histomorphological score of repairing cartilage defect of knee joint in rabbits decreased significantly in Kangfuxin low-dose, medium-dose and high-dose groups (P<0.05). At 12th week after medication, compared with model control group, the histopathology degree of cartilage defect of knee joint in rabbits was significantly alleviated in Kangfuxin low-dose, medium-dose and high-dose groups. At 4th, 8th, 12th week after medication, compared with model control group, the levels of NO and LPO in joint fluid and PYD level in urine were decreased to different extent in Kangfuxin low-dose, medium-dose and high-dose groups, while SOD level was increased to different extent; at 12th week after medication, the difference of each index has statistical significance (P<0.05 or P<0.01). CONCLUSIONS: Kangangxin liquid can significantly repair cartilage defect of KOA cartilage defect model rabbits, the mechanism of which may be associated with increasing the expression of SOD and mediating NO-inhibited chondrocyte apoptosis.

ABSTRACT

BACKGROUND:Acel ular tracheal matrix is similar to the native trachea with structure and biological performance preserved after decel ularization, and it is an important aim in tissue engineering to find an effective method of decel ularization. OBJECTIVE:To select the optimal decel ularization method through comparing chemical extraction method with detergent-enzymatic method for preparing rabbit tissue engineering trachea matrix. METHODS:Thirty tracheas from New Zealand white rabbits were randomly divide into three groups. Twenty of rabbit tracheas were subjected to decel ularization using 2%TritonX-100 combined with deoxyribonuclease I and ribonuclease (chemical extraction method group), and sodium deoxycholate combined with deoxyribonuclease I (detergent-enzymatic group), respectively. The other ten were given no intervention as controls. Samples were col ected and observed by hematoxylin-eosin staining, Masson-trichrome staining, safranin O staining, DAPI staining and scanning electronic microscope. RESULTS AND CONCLUSION:Hematoxylin-eosin staining demonstrated that compared with the control group, almost al cel ular components of the mucosal epithelium were removed in the detergent-enzymatic and chemical extraction groups, and there were few remnant chondrocytes. Masson-trichrome staining indicated that compared with the control group, components of the mucosal layer chondrocytes in the chemical extraction and detergent-enzymatic groups were completely removed, with only part of remained chondrocytes in the cartilage zone. Glycosaminoglycan was slightly decreased both in the chemical extraction and detergent-enzymatic groups shown by Safranin O staining, but more reduction was found in the chemical extraction group. DAPI staining reveled that only a smal amount of cartilage cel s remained in the dense layer of cartilage and lacuna both in this two methods. Scanning electronic microscope showed that using the detergent-enzymatic method there were the hierarchical structures of native trachea, but slight disruption using the chemical extraction method. In conclusion, decel ularized rabbit trachea matrix obtained by detergent-enzymatic method is better, with little disruption to the matrix.

ABSTRACT

Objective To analyze the clinical value of sTNFR-1,sIL-2Ra and IGF-1 on depression in patients with esophageal cancer by comprehensive nursing intervention and explore the possibility of serum indicators to replace DSI and HAMD. Methods 120 patients with pathologically confirmed esophageal cancer were included in the study.Zung depression status inventory (DSI)and hamilton depression rating scale (HAMD)were used to assess depression.Serum sTNFR-1,sIL-2Ra and IGF-1were detected by enzyme-linked immunosorbent assay (ELISA).Results 69 patients were classified as anxiety and depression group,51 patients were as normal control group.There were 32 patients with depression.Serum sTNFR-1, sIL-2Ra and IGF-1 levels in study group was higher than that in the control group (P<0.05)and these serological indexs increased from the minimal depression,mild-to-moderate depression group to severe depression group (P<0.05).Conclusion Serum sTNFR-1,sIL-2Ra and IGF-1 levels may be used as evaluation index of nursing intervention,but need further clini-cal validation.
